Connect Form to CMS Collection

Hi there,

I am creating a CMS template page with a form, like that people can subscribe to participate in a specific activity (Lab). The goal is that when they fill up the form, we can know what activity they choose.

I have the same question about another website. It’s a store and what we need, is that when visitors are on a product page, they can book it.
