Duplicate ID and form button not working

I’m having issues with my newsletter form button not working. I have checked all the settings and everything seems to be fine. I have an error code of Duplicate ID and this might be the issue. I basically have the newsletter form and sign-up form on the same page.

Any tips from you guys I would greatly appreciate. thank you so much,
