Identical, duplicated form works on one site but not another

We have two sites for different locations. I’ve duplicated a form element that works perfectly on one, but won’t submit correctly on the other.

Nothing occurs when hitting the form submit button.

Could anyone please troubleshoot this for me? I’ve likely missed something glaringly obvious.

It’s your custom code setup.
You’re likely missing some form of identifier that your code relies on.
