Customize forms?

I need to be able to do something like this:

How did you hear about us?
DROPDOWN

  • one
  • two
  • three
    SUB dropdown under three
    • a
    • b
    • c
  • four
  • Yes or no?
    Drops down for no: If no why? _______enter why here.

Is this possible?

I think with one dropdown it will not possible. Maybe you can think about the combination of radio buttons and dropdowns?

@sabanna so if someone were to pick three i can’t make something pop up saying to select something?

Or when someone selects no, an area to type in why?

Ok, now it is a bit hard to understand what you are trying to archive. Could you create some kind of prototype of show us an example?

here is my form right now:

So when someone selects how they heard about us, let’s say they choose search engine. I want another sub category to pop down saying: - Google, - Bing, - Yahoo…

or if someone selects social media, another sub dropdown saying: - Facebook, - Twitter, - LinkedIn…

Then if someone chooses yes or no for credit cards, if they pick no, a section pops up saying If no, do you plan to in the near future, have it say a yes or no.

Ok, I have some ideas, but need a time for creating a work example. I think it is possible to make combination of radio buttons and dropdowns.

It’s free! for a certain # of submissions. But honestly I have always gotten away with the free version lol.

@AntonioBalderas This looks great too but my manager is looking for something that doesn’t cost additional and by looking at the plans for this we would need to purchase to be able to do what I’m looking for! I appreciate the suggestion though!

@ctrav - they have custom CSS options. So if you know CSS, then you can edit and style it any way you’d like. :grin:
