Hi @rileyrichter, have you published the site and also submitted at least 1 form submission prior to trying to setup the Zap?
Do you have more than one form on the page with the same name?
I would recommend to publish the site, send at least one form submission and check the form names are unique. In Zapier, create an entirely new Zap and see if the behavior persists.
I have seen this error, but usually by checking these items above and re-creating the zap will help to solve the issue.
