has anyone ever done something like this, I’ve come across something like this on a couple of websites and now I’d like to create a request for quotation page myself where I need something like this

I typically use a stack of Make, AirTable, and MailJet to build this type of setup. That will get give you the pieces for;
If you want the user to be able to see “results” on the site, that’s more work and if those results need to be secure, that’s a fair bit more complex.
I’d probably recommend you look at Wized+Xano if you take that direction.
