Hi guys, can anybody recommend any apps that will allow me to build a custom questionaire / survery for my users to submit, leading to a CTA page.

In short, you have a ton of options, it really depends on the details;
- How much design control you want over the UX
- How much simplicity you want in building and maintaining this piece
- The functional UX of the form ( multi-step, slider approach, vertically expanding steps… )
- What happens after the form is completed, e.g. email/lead capture, follow up steps
Your best bet is to know your options to help you solidify that, try out Typeform demos, Jotform demos especially to get a sense of their strengths and weaknesses.
