Form on CMS item

I have a form on a cms item. the form info goes to airtable does anyone know how i can tell which cms item the form was submitted on???

Add slug or cms data to form submissions

Just add an embed code with a link/button and dynamic variable like this. (hide the referrer field if you don’t want to display it to the user). Blog Post 1 Blog Post 2 Otherwise, JS can also grab the referring page automatically using location.referrer.
