Submit button not working

Hi everyone,

Navigate to the Momentum Application

2 problems:

• Submit button doesn’t acutally submit, or do anything, it just goes back to slide 1?
• Rules like EMAIL or CELLPHONE or REQUIRED are complete ignored and bypassed?

What would be the main reasons that a form would be this disfunctioned when cloned?

Any help would be appreciated
