My form only works on the contact page

Hello,

I created a form using some custom code to show the errors and it is working just fine on contact page (Sofist: conte para nós o seu desafio). However, it doesn’t works on any other page. My intention is to use it on severals landing pages, just copying it and changing the Form Name (Sofist: soluções em Qualidade de Software). I even made a test not using a folder and doesn’t works either (Soluções Teste).
