Happy to help - but I think I would advice to go with jotform.com - they offer exactly those functionalities you need and you can design it the way you want! After creating the form you can simply embed the form using a code in your webflow site.

I know jotform very well. But I like to keep my design and font for this page. I someone can copy the page 1:1 to jotform that works for me too. http://yachtsigns.webflow.io/free-quote
