Javascript to auto populate form from excel data

Hello, I’m trying to create a form that will auto populate using Excel data.

The end result would look a lot like this: Edit fiddle - JSFiddle - Code Playground
but instead of making a table it should populate the inputs in a form.

I’ve created the layout, but don’t know how to go about coding the solution.
