Help building a form

How should I go about making these forms?

It looks like she needs some eCommerce support on her website. Those pdfs ask for credit card and personal information which needs to be done in a secure environment.

@AlexN Customer doesn’t want that part included on the form. Here’s what I have so far

Can you help me figure out why when you select the item the drop downs don’t work like I’m wanting?

@AlexN So example they can click on T-shirt, sweatshirt, shorts, or pants. They can also choose more than one item; but when they do select an item the dropdown appears with more choices for that particular item. It doesn’t appear to be working right and when you select more than one item the form looks all funky!
