Form Fields Issue

I’ve set up a form with three fields:

When my client receives a form submission it arrives like this:

Why are there three fields in the beginning and how can I make them go away?

That’s super weird! I’m not seeing anything in there that would cause that.

Have you tried deleting the form and re-adding it?

@Cricitem not tried that yet. Would prefer to avoid it if possible.
