Trying to set up a custom domain. Is the attached photo correct?

Almost. 
Under the Name column, change bluebellsboutique.com to “@” without the quotes, and remove the URL from the CNAME record so it simply says “www” without the quotes. So the Name column from top-to-bottom should read:
@
@
www

Thanks buddy. Unfortunately it is simply not letting me change them and i dont know why. I will contact the domain service provider for their help.
