Conditional Form

Hello Guys,
I have a problem creating a conditional form.
I have 2 select lists (countries and type of user)

Example:

Please select… v

Germany
France
USA
Italy

Please select… v

Private
Institutional

(OK)

What I want to achieve is: have a different link based on selections

If I choose Italy and Private then go to page A
If I choose USA and Institutional go to page B
If I choose Germany and Institutional go to page C

… and so on.

How can I do that?
