Bespoke Form with MailChimp

Client want a bespoke form integrated with MailChimp - I have set up a form in MailChimp and assigned the same ‘names’ to the form fields in Webflow and also with the drop downs and checkboxes.

The form isn’t working but I can’t find out why? Can anyone help with this?

Thanks for the video, I have added the action set up already? Could it be anything else causing the issues?
