[if condition] drop down form to show/hide another form?

Hi Webflower!

I just made a contact form which I put 2 subject on dropdown form to let user choose for what they want to contact us:

  • Book the services
  • Ask about the services

Right now, I put the reservation date input form in both subject,
which is confusing when people choose “Ask about the services”.
Because there is nothing to do with the reservation date

So How to:

If user choose “book the services”
then reservation date input form is appear

but if the user chooses “ask about the services”
then reservation date input form is disappear
