Setting up a form to respond when radio button is selected

I’m trying to set up a returns form for my children’s eCommerce store.

I’m having troubles with the radio buttons.

How do I set up an interaction showing different questions once they select a different radio button option? If that makes sense.

For example, if they press faulty item. The questions shown will change to what’s related to their selection.
