Interaction for each Collection Item not all

[TN_Thien_Tri (TN Thiên Trí)]

I want the form show up only for 1 Collection Item when I click the Red Button, but now it apply to all item in that Collection List.

Is there anyway for me to do that? Code is ok. All I need is it work.
