Contact form works successfully on Chrome, but not safari (it forwards to /undefined)

Forms submit successfully in Chrome, but results in an /undefined URL in Safari. I’ve looked into the console and can’t find anything that would be causing this.

Any ideas?

Just wanted to bump this up a bit. I’m still scratching my head about this, and have yet to come up with any solutions.
