When Collection Item slugs are used and when they are not

Am I right in thinking Collection Item slugs only become useful when there are Collection Item Pages? And if a Collection only gets uses in a Collection List the slug is not used?

From the perspective of a web dev, yes, that’s roughly correct.
The Collection Page will still get created, blank, but if you’re never linking to those items, than it’s contentless and irrelevant.

That said in most cases, it’s worthwhile for SEO and UX purposes to build out those Collection Pages, if only because they’re linked in the sitemap.

Could this SEO issue be sidestepped by setting the CMS template page to “Exclude these pages from site search results” for Collections in which all items don’t have Collection pages?

Yes, but there’s not really an issue with those blank pages, it’s just a lost opportunity for improved SEO. If you really have nothing to put there, just add a META noindex;
