Changing the name of the form seems to work(removing spaces).
Haven’t verified this though.
Bubbling this back up. I am experiencing the same issue with a client site. I have five forms using Logic to send customized emails on form submission. Every other form works fine. This one worked great up until Feb 21, when any submissions stopped working, and we began to get “Your Flow has been disabled…” emails. I can test the flow in Designer, and it works great. But no submission will work as soon as it’s published (in both Staging and Production). I have tried:
- Testing and republishing the Flow to Staging and Production.
- Verifying that all form fields were the correct type, then republishing the Flow to Staging and Production.
- Renaming the form, re-establishing the trigger in the Flow, reconnecting all the email variables, and then republishing to Staging and Production.
- Removing any unused script on the form page.

Same result. The Designer test works, but the Live form doesn’t.
