Hello, we have a form on our website that was made by our website designer. Here it is: Medical Intake Form
It was working fine for months but in the last week, we are no longer getting SOME of these forms. The content for these missing forms that were filled in is not found in the Forms section of our Webflow account, and no emails alerting us to these forms are being sent to us. We know the forms ARE being filled out because the only way clients can upload pictures, docs, etc to us, is through this form. We are using uploadcare.com to receive the uploaded pics, docs, and are receiving them. But again, not ALL forms are coming through to us anymore.
We’ve tested this form ourselves using IE 10, Safari and Google Chrome and we have been getting the forms. Any advice would be really appreciated.
