Form Recommendations

I’m looking for recommendations for a Form App that could

• log the source url (e.g. I could see the Google Ads source link)
• Send form submissions to different email addresses
• Send form notifications back to submitter (or integrate with an email service like MailerLite).

I’d do this with script on the entry page, and possibly use webstorage to track that up until the submission occurs.
