Form not Submitting in live site HELP!

I just built a new form for a ministry and I’m getting a submission error. Please take a look! Its the connect card form connect with us page.

OK. What is the submission error?

Try changing the name value of “No” under First Time Visitor to something unique and test again.

Same error. I changed it to returning viewer. Can Webflow accept the check boxes and text boxes? This is my first time using this multi functionality.

@Matthews_Tech - I was referring to the ID and Name of the element which is the same as “Yes”, not the label. They should be different on each element.

Where do I find that. In the settings? Where it says ID?

Thanks for the insight. I gave every Id a number and still an error.
