Doublons on my forms - 1 submission equals 2 leads in duplicate

I have Doublons on my form. It appeared spontaneously without any change on our side.

Could you help me fix the issue ?
It appeared from nowhere like 2 or 3 days ago for no reason. Does anyone experience the same thing ?
