Display form animation won't work on published website...?

I’m trying to create a ‘show form’ animation that’s triggered by hitting an apply button. It works in preview mode but won’t work on the published site. What gives?? Does anyone know why this is happening?

@cyberdave do you or any other webflow staff whats going on here? I’m trying to get this out today and can’t figure out what’s going on with this form. Please help. Thanks!
