Checked state of checkbox not editable

Hi,
I’m trying to add checkboxes to a form and have images instead of the actual checkbox. And every state has different images. This work fine for the first 2 checkboxes as you can see in the read only link.
But for the 3rd checkbox I did the same thing but for some reason I can’t edit the checked state. It just wouldn’t select it. As you can see in the screen capture video below:

What am I missing?

OK I changed the combo classes to a single classes per checkbox and than it does select the state. Why the first 2 were working and the 3rd and 4th didn’t, I have no idea, but it works now.
