Checkboxes to google sheets

Hi there

I am struggling to transfer responses from input label checkbox to google sheets.
I tried using zapier, but I don’t know how to assign the value “true” or “false” if the user selects or doesn’t select each input label. Can someone help me assign boolean values based on if the user selects each input label?
